Insurers blame AI coding for $942M in added health spending

Patients are being documented as sicker than their treatment suggests, and the Blue Cross Blue Shield Association blames software on the hospital side of the claim.

Its analysis covers two years of spending and puts the extra cost at $942M. Reviewers found a sharp rise in records listing complex conditions, alongside what the association called a clear disconnect between medical coding and care actually delivered. They saw no matching shift in treatment.

The New York Times described the finding as the latest sign that AI is contributing to rising healthcare costs, and noted that automation now sits on both sides of the payment argument.

Not everyone reads it as a fight. Dr. Shiv Rao, founder of the clinical documentation company Abridge, said the trajectory could produce a future nobody wants, with bots arguing against bots and agents against agents. Handled differently, he argued, the same tools could cut costs and cool the dispute.

Luke Chalker, a senior vice president at the association, rejected the framing of a fair contest. He called it a completely one-sided blood bath, with insurers on the losing end.

Payment fights between hospitals and insurers go back decades. What is newer is how quickly records can be generated, and how hard it becomes to separate a real diagnosis from a billing pattern.
